Conceptually, the holographic doctor's actual "body" is the holographic emitter itself, no?

So Self Surgery would have to be performed on the holographic emitter unit with primitive tools.

I think Dr. Crusher is better suited to this. At the end of the day, the EMH is a stop gap that grew into something more. Dr. Crusher is on starfleet's flagship.

It's worth pointing out that primitive is subjective and that a modern state of the art surgery ward would be primitive to both competitors in this match.

We also have to acknowledge that the Doc being a hologram isn't going to work on himself the same way and organic life form would.

That said The Doctor has worked on his program/Holo-emitter/power source before. To give himself upgrades and to keep them running and he has done so while Voyager was damaged. He has also effected repairs on his mobile emitter a device that is from 500 years in the future of his universe. That's the equivalent of fixing a smart phone in medieval black smiths shop and it's the equivalent of surgery for the Doc.
